Hardcore ve metal crossover tarzında olan San Diego'lu grup 2001 yılında, Tim Lambesis, Evan White ve Jordan Mancino olarak kuruldu. Hemen o yılı Haziran ayında Pluto Records'tan ilk albümleri olan Beneath the Encasing of Ashes'i kaydettiler. Ardından hemen turlara başladılar. Ağustos 2002'de yine San Diego'lu bir grup olan American Tragedy ile yaptıkları split CD ile de ileride oluşacak olan başarılarının işaretini verdiler. Turlarına devam ettikçe Metal Blade şirketinin de dikkatini çektiler ve Mart 2003'te bir anlaşma imzalayıp Frail Worlds Collapse albümünü kaydettiler.
